We are happy to register new NHS patients within and outside* our practice boundary area. *Some restrictions will apply to patients who live outside our practice boundary area.
How to Register
Adults and 16 and over
To register with our practice, please follow the links below to complete the online registration forms. You must complete both forms below.
If you wish to register with the practice but DO NOT live within the practice boundary please contact reception for further information. Please note that we only accept patients who are aged 16 and over as out of area patients.
After you have completed and submitted the form you must either email (email@example.com) / drop in our post box/ come in person within 7 days .
Everyone is welcome in general practice. You do not need to proof of ID, address, immigration status or an NHS number in order to receive care or see a GP. We only ask identification to confirm identity and that you are living at our catchment area
Acceptable identification includes:
- Birth Certificate
- HC2 Certificate
- Rough sleepers’ identity badge
- Hostel Registration/mail forwarding letter.
- Utility bill
- Council Tax

Registering Child (under 16)
If you are registering your child please complete the following forms
After you have completed and submitted the form you must either email (firstname.lastname@example.org) or drop in our post box with the copy of the red book.
